To find out if you qualify for the sliding fee discount, you will set up an appointment with our Member Services Specialist. You will have to provide proof of income verification and other required documents. These include Income Tax forms, check stubs for the past 30 days or letter from employer that mentions your salary and hours worked. The requested information must be provided to the Member Services Department before your appointment date in order to have any sliding fee discounts applied to that visit. The sliding fee program cannot be applied to appointments prior to the applications effective date.
It is very important that payment is made at the time of your visit. We accept cash, personal checks, money orders and credit cards (VISA/MasterCard).
